*Due to COVID, we are now taking cancellations through email with the form or docusign or by requesting a docusign over the phone.

We understand that there are many reasons for canceling membership. Any cancellation must be submitted in writing and in person. We cannot accept this form via email or accept cancellation over the phone. Please download the PDF attachment below, complete and bring it to the yoga studio at your earliest convenience. Please note that with this application for membership cancellation you are submitting a thirty (30) day notice to cancel.

If you have any questions about your membership, or you would simply like to adjust or have more options please contact our front desk, and we will do our best to accommodate your needs.

PRIVACY POLICY

I Am Yoga is committed to protecting the privacy of your personal information. This policy summarizes the type of information that we collect through our web site and how we use, or might use, that information.

By using our web site, you consent to the collection and use of information collected on our website. In order to keep you informed about our services, we may collect personal information from you, including name, title, company name, home and/or work addresses and telephone numbers, and email address. We do not use spy ware, or similarly intrusive software.

We collect the personal information described above in order to market our services directly to you by sending emails, advertisements and updates about areas in which you have expressed an interest. Please notify us in writing if you do not wish to be marketed to by I Am Yoga. We may also provide your contact information to third parties that perform marketing functions for I Am Yoga. If we do share your personal information such third parties, we will specify that they are not to use your contact information for any other purpose.

I Am Yoga reserves the right to disclose your personal information in response to legal process, such as a court order or subpoena. Furthermore, any personal information you provide may be transferred in a merger, acquisition, restructuring, or other corporate transaction involving I Am Yoga. We will not share your information with any third party except as set forth above.

I Am Yoga reserves the right to change, modify or replace this Privacy Policy at any time within our complete discretion. Changes will be posted here before the new, changed or modified policy becomes effective. It is your responsibility to periodically review this page to remain informed of our current privacy policy and procedures.

Our web site may contain links to third party sites. I Am Yoga has no control over the privacy policies and collection practices of those sites, and is not responsible for their content, collection and use of information, or freedom from errors or viruses. It is your responsibility to know when you are linking to a third party site, and to inform yourself of the privacy policy and information collection practices of those sites.

FREEZE POLICY

Members can freeze their membership for up to 2 months with a minimum of 1 month. Members can freeze their account up to 2x a year with at least one month in between suspensions. Benefits of freezing your account:

You will keep your rate locked in
You will not be charged your monthly rate
Your account will pick up where you left off

While your account is suspended you will not be able to use your membership or the facilities. Once your suspension is complete your account will continue to be charged at your regular membership price. Memberships cannot be retroactively suspended. If you wish you cancel your membership, the 30-day cancellation policy will be applied.

COVID 19 AND SAUNA

LIABILITY WAIVER AND RELEASE

I acknowledge the contagious nature of COVID-19 or any other virus, bacterium or contagion that is capable of inducing physical distress, illness or disease and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 or any other virus, bacterium or contagion that is capable of inducing physical distress, illness or disease by attending any event at I Am Wellness Studio (the Studio) and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 19 or any other virus, bacterium or contagion that is capable of inducing physical distress, illness or disease at the Studio may result from the
actions, omissions, or negligence of myself and others, including, but not limited to, Studio employees, teachers, independent contractors and other participants. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself and/or my
children (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I and/or my child(ren) may experience or incur in connection with our attendance at the Studio. I hereby release, covenant not to sue, discharge,
defend and hold harmless the Studio, its employees, agents, and representatives independent contractors, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release and
indemnity agreement includes any Claims based on the actions, omissions, or negligence of the Studio, its employees, agents, and representatives, whether a COVID-19 infection or any other virus, bacterium or contagion that is capable of inducing physical distress, illness or disease
occurs before, during, or after participation in any Studio program.

The health and safety of our community is our top priority, and we ask for your assistance in keeping yourself and others safe and healthy. The I AM YOGA Wellness Studio is taking steps to protect our community and in addition, there are steps whereby each person can take charge of their own health and wellness inside our studio. We look forward to continuing to provide a safe and healthy yoga/fitness environment to our beloved yoga community. We believe yoga/fitness is a safe and wonderful way to stay healthy. We hope that you will continue your yoga/fitness practice with us, keeping our
yoga/fitness community thriving and strong.

1. STAY HOME If you exhibit flu-like symptoms such as fever, cough, shortness of breath or breathing difficulties or a rise in temperature, please stay home! Keep our community safe. Your temperature will be taken prior to admittance and you will be turned away with no refund if your temperature exceeds acceptable limits.

2. SURFACES: Our efforts to keep our studio germ free include cleaning and sanitizing countertops, door handles, bathroom fixtures, yoga floors, and often-touched areas every day. We use UVC lamps at night to sanitize the entire studio. Please wash your hands before and after class.

3. PROPS: All community props are off limits until further notice. Students are responsible for personal mat and desired props.

4. WATER: The water dispenser is also off limits until further notice. Water bottles are permitted in class but cannot be filled from our water station.

5. REGISTRATION: Students are REQUIRED to sign up for class via MindBody prior to arriving at the studio. All Payments are to be processed on line thru MindBody prior to arriving. If you need a discount code please contact the studio. This is to protect the students as well as the staff when processing check in. No “walk-ins” will be permitted.

6. SIZE OF CLASSES: Classes will be sized to 50% capacity per the states of California’s Covid 19 directives. If you register for the class and then cannot
make it, please go back in and cancel.

7. PPE: You must wear your mask into the studio and once on your mat, you may remove your mask for the class in order to prevent lack of oxygen or
hyperventilation. Once class is over you must return to wearing your mask until you exit the Studio.

8. PLACEMENT OF MATS: Safe distancing practices are in order. Our floor is marked off to ensure that mats are 6-8 feet apart. I AM YOGA SAUNA WAIVER OF LIABILITY (During Shelter in Place no sauna usage)

1. The use of drugs, medication or alcohol prior to or during the sauna session may lead to dizziness or unconsciousness.

2. Please consult your physician if you are in doubt of your ability to use the I AM YOGA sauna for health reasons.

3. No clients under the age of 18 are permitted in the sauna unless accompanied by a supervising adult.

4. Please discontinue the use of the sauna if you feel light-headed, dizzy or heat exhausted.

5. Sauna sessions should be limited to a maximum of 30 minutes and temperatures must stay below 150f.

6. It is advisable to drink plenty of water before and after a sauna session. Water bottles are not permitted in the sauna.

7. It is advised not to eat at least one to two hours prior to your sauna session to avoid any ill feelings.
8. Clients using any medications must consult a physician or pharmacist prior to the use of the sauna.

9. Pregnant women should consult their physician prior to the use of the sauna. Excessive body temperatures have a potential for causing fetal damage during the early days of pregnancy.

10. Do not use any chemicals or lotions prior to your sauna session. These items may block pores and affect perspiration as well as stain the wood of the sauna.

I acknowledge and accept the risks inherent in the use of the I AM YOGA STUDIO and Sauna. I voluntarily assume the risk of injury, accident or death, which may arise from the use of the I AM YOGA STUDIO and Sauna. I and any of my heirs, executors, representatives or assigns hereby release from all claims or liabilities for personal injury or property damages of a kind sustained while on the premises, during the use of the I AM YOGA STUDIO and sauna and from any advice provided by an employee, independent contractor or any representative of I AM YOGA. I agree that this Application and Waiver is in effect for all I AM YOGA STUDIO and Sauna sessions and will not expire unless requested by either party.

By signing below I acknowledge that I have read the foregoing Liability Waiver and Release and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; that I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Covid 19 and Sauna Liability Waiver and Release as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.

In checking the box on the form below, entering, and watching the videos in the Online Library of I Am Yoga Wellness Studio I understand that classes at times may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will adjust the posture and ask for support from the instructor. I will continue to breathe smoothly.

Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. I affirm that I alone am responsible to decide whether to practice yoga. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against I AM YOGA Wellness Studio, 2031 Geer Rd. Turlock, California. We recommend seeking doctors approval before starting any new physical practice.

Acknowledge of Understanding: I have read this waiver of liability and indemnification agreement and fully understand its terms and my assumption of risks involved. I understand I am giving up substantial rights, including right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest allowed by law in the State of California.

You expressly acknowledge and agree that by electronically signing or otherwise electronically accepting our to this Release you are relinquishing all rights you may have to sue I AM YOGA for injuries arising out of the use of any I AM YOGA facilities or its services. This Release is intended to be interpreted as broadly as allowed under the law of the State of California.

I agree and understand that classes and activities at I AM YOGA WELLNESS STUDIO (the Studio) may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death both within our virtual and in-person classroom. I hereby waive and release any and all claims that I may now have or may have in the future arising out of my activities at the Studio and that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against I AM YOGA WELLNESS STUDIO its employees, agents, representatives or members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise. To the extent that such claims are made I hereby covenant to defend and hold harmless, the Studio, its employees, agents, and representatives independent contractors, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind including attorney fees arising out of or relating thereto.

On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand that yoga and fitness classes include physical movements that involve the risk of injury, even serious or disabling. I agree that it is my responsibility to notify the instructor of any physical injury or other condition affecting my ability to practice yoga/fitness classes at I AM YOGA whether they be pre-existing or occurring during class and that I will inform the instructor immediately if any injury does occur during class. Instructors are not responsible for your medical care. Instructors will assist you and follow your directions regarding medical assistance for any condition or injury you suffer on the premises but are not responsible for the necessity or propriety of the treatment or assistance you receive.

This Release is intended to be interpreted as broadly as allowed under the law of the State of California.

We appreciate your business!

Memberships, Drop-Ins, Passes & Sessions are non-refundable and non-transferable. Our schedule and rates are subject to change. We offer limited extensions and holds on membership packages for medical reasons only with a written doctor's request. A written 30-day notice is required to cancel an auto-pay membership. If you have purchased a series, please note the expiration date. Series will expire 1 year after purchase without use. Private sessions require a 24 hour minimum cancellation notice to avoid a $30 cancellation fee.

All lululemon and retails items may be returned for a full refund within 30 days with a receipt and tags still attached. After 30 days only studio credit will be given.
